If you have questions about aging or living with a disability, your local aging and disability resource center (ADRC) can help.
The ADRC provides information on a variety of programs and services. That includes long-term care options.
The ADRC also helps people apply for programs and benefits, including publicly funded long-term care.

If you or someone you know is experiencing issues with memory or having problems with their cognition, an important first step is to have a memory screen completed. This can be done by a primary care physician or through your county Aging and Disability Resource Center.
In addition, the Wisconsin Alzheimer's Institute has a network of providers to help you or your loved one receive a diagnosis.
Maintains a network of affiliated dementia diagnostic memory clinics who are committed to following best practice guidelines established by the WAI. Network clinics use a team of dementia care experts to do high-quality, thorough, interdisciplinary evaluations.
The WIA Clinic Network has 40 affiliated Dementia Diagnostic Clinics, including clinics in Milwaukee serving African-American and Latino communities. These diverse, state-wide diagnostic clinics provide quality care to more than 3,000 new patients annually, many from rural, often under-served parts of Wisconsin. Respite Care Association of Wisconsin
Provides a registry of trained respite care professionals and offers grants to cover costs of receiving respite care. Grants may cover occasional respite care to give caregivers a break or provide supportive services like housekeeping or lawn care.
Mosaic Family Health REACH Program
Resources for Enhancing Alzheimer’s Caregiver Health (REACH), a program of the Rosalynn Carter Institute for Caregivers is a unique evidence-based program for individuals caring for a loved one with memory loss.
REACH brings current information, 1:1 support and strategies for coping with troubling behaviors, while improving a caregiver’s emotional, social and physical health.
Meetings with the caregiver are provided at home for up to 12 sessions over a 6-month period. Each meeting offers opportunities for in depth education, problem solving, stress management and support while improving caregiver confidence.
Mosaic Family Health - Family Caregiver Support Group
The Family Caregiver Support Group is designed for anyone identifying as a family caregiver, operates in an open format with no RSVP needed, and offers a call-in or virtual attendance option.

Outagamie County Caregiver Coalition - Caregiver Chats
Caregiver Chats are designed to provide a place to openly share and discuss life as a caregiver. All caregivers can attend regardless of county of residence.

Thompson Center on Lourdes (TCoL)
The Thompson Center on Lourdes is a nonprofit organization that provides opportunities for people 50+ to participate and connect through enriching activities in the Fox Valley. The center focuses on fighting the effects of loneliness, isolation and depression in seniors by offering opportunities for education, socialization, wellness, and collaboration.
Options for Independent Living
Options Dementia and Memory Loss Services help people improve their health, safety, and independence by connecting them with information, resources, adaptive equipment, and tools to remove barriers and live the life they want.
Home Visit Assessments:

You'll recieve specific recommendations to remove the barriers and improve accessibility for maximum independence.

Rebuilding Together Fox Valley
Rebuilding Together Fox Valley (RTFV) is a nonprofit organization that makes critical home modifications and repairs to income-qualified homeowners and nonprofit organizations, at no cost to them. RTFV volunteers make homes safe and healthy, and they help individuals age in place.
Rebuilding Together Fox Valley (RTFV) provides a free home modification assessment, conducted by a certified staff member. The assessment identifies challenges in the home and offers recommendations to improve safety, independence, and daily living.
Recommendations may include adding or adjusting equipment, technologies, or features to improve the home's layout and functionality.
After completing a home assessment, Rebuilding Together Fox Valley creates a scope of work outlining the modifications and repairs they can provide for homeowners at no cost.
Fox Valley Memory Project works to create dementia-friendly communities where the fear and isolation often associated with dementia are eased by connecting people with one another and with resources to live well with dementia.
